Create an approval request directly

Raises a request with no policy behind it: PENDING,
sourceType: MANUAL, initiatedBy the caller's sub.

Every approver expression is resolved before anything is written,
so the request is created whole or not at all. An unresolvable
expression is a 400, as is an approver equal to the caller (unlike
the execution path, which prunes the initiator and carries on).

An omitted reason is stored as "approval requested manually"; an
omitted expiresAt never expires, and a past value is a 400. There
is no idempotency on this endpoint.

date-time

When the request stops accepting decisions. Must be in the future;
an instant at or before now is a 400. Omit for a request that does
not expire; nothing will then sweep it.

The version of the Quant Flow API the request follows, as major.minor.patch (for example 1.0.0). The request is interpreted, and the response shaped, according to that version.
